There is a cause to what happened and more than likely, it was an innocent error that snowballed into something bad. It doesn't mean the person won't be blamed for it. Right now there's too many variables to say with certainty what happened.

Could have very easily lasted days/weeks if they didn’t have an EXTREMLY up to date backup of the database. They must have suspected the update could break things. All in all, they deserve a bonus and paid time off.
